If you would like to request copies of your student's records related to special services, please fill out the form linked below and return it by mail or email to:

Destruction of Student Records
The Hunterdon Central Regional High School will be destroying special education files of all students classified as eligible for special education and related services and/or classified as eligible for speech/language services whose graduation year is 2020 and birth year is 2002. For students who are Medicaid Eligible, destruction of files will occur for those whose graduation year is 2018 and birth year is 1997.
Should you wish to obtain these records, it is important that you complete an Acquisition Request for Permanent Transfer of All Child Study Team Records form, which can be downloaded below. You should mail the fully completed request to:

The request must include a phone number and an original signature of the adult student or legal guardian of the adult student. Bernadette Burns or Louise Kubida will contact you to confirm that we indeed have a file and tell you when the file will be ready for pick up or give you the estimated cost of mailing the file if you choose not to pick it up. Requests must be made no later than April 30, 2025.
