f your child needs working papers, please schedule an appointment by emailing workingpapers@hcrhs.org Before coming in, please visit the website and download, complete, and print the working papers form found on the Working Papers Webpage

Please note that all sections - A through C for enrolled HC students and A through E for students enrolled at "other" schools - must be completed prior to certification. Hunterdon Central is the last step in the Working Papers process. Working Papers are processed on the 11/12 Campus (off of Junction Rd.).

New Student Physical Forms & Requirements

New Jersey State law requires that each student must be examined at their medical home (family doctor) upon entry into a school district. Therefore, Hunterdon Central requires a physical examination for all Grade 9 students, as well as new students in any grade level. In addition, one physical per year is required for students participating in athletics. Please note that a sports physical will fulfill the requirement for a freshman physical or a new student physical.

The Pre-Participation Physical Evaluation Form must be used for all Grade 9, new student and athletic physicals. Forms can be downloaded and printed from this webpage; please click on the link Health Forms. Forms can also be picked up in Health Offices, or can be mailed to student homes, upon request.

Grade 9 and new student physicals must be turned into Health Offices by September 30, 2019. PLEASE NOTE: any students who have not submitted a Grade 9/new student physical to Health Offices by the due date will not be allowed to return to school until the physical has been completed and submitted.
