Proof of Residency and Freshman Physicals

Proof of Residency and Freshman Physical Forms are due to the Freshman House Office by September 29, 2023. Any student who has not met these requirements by October 1 may not be allowed to attend school until the proper forms are submitted. Please use the following information to submit Proof of Residency and Freshman Physical Forms:

Proof of Residency Information:

Attendance at Hunterdon Central is approved for all students who are living in Delaware Township, East Amwell Township, Flemington Borough, Raritan Township, and Readington Township, or for those students who have been accepted as Interdistrict School Choice students.

In order to ensure compliance with local residency regulations, all students in the Class of 2027 are required to present proof of residency documentation to the Freshman House Office.

To establish Proof of Residency, please fill out the Proof of Residency Form and submit it to the Freshman House Office with a copy of two items from the required Proof of Residency Documentation list. The form and documents may be dropped off or mailed to:

Copies of Proof of Residency documents may also be dropped off in the red mailbox in the vestibule of the 9/10 Building (Fountain Entrance, off of Route 31.)

Freshman Physical Information:

New Jersey State law requires that each student must be examined at their medical home (family doctor) upon entry into a school district. Therefore, Hunterdon Central requires a physical examination for all Grade 9 students. Please note that a sports physical will fulfill the requirement for a freshman physical.

Physical forms may be downloaded and printed for our website, here: Health Forms The Pre-Participation Physical Evaluation Form must be used for all freshman physicals.

Forms can also be picked up in Health Offices or can be mailed to student homes, upon request. Freshman Physical Forms must be turned in or mailed to the Freshman Health Office by September 29, 2023. If you have any questions about Freshman Physicals, please call the 9/10 Health Office: 908-284-7304.

All students who require medication during the school day, either prescription or non-prescription, must have a physician's order on file in the appropriate Health Office. Care plans and medication authorization forms can be found on the Health Forms webpage. Please have these completed by your child's physician and return them to the appropriate Health Office.

Failure to comply will delay your child's participation in sports, field trips, and appropriate nursing care. Please note that physician orders for medication must be renewed for each new school year.

Important News & Dates for Incoming Freshmen

If your child is interested in playing fall sports at Central, please be advised that the deadline for Fall Athletics Registration is on July 7.  You can view detailed information about the registration process on our Athletic Registration Webpage. If you have any questions, please contact our Athletic Director Jesse Spencer: jesse.spencer@hcrhs.org    

Counseling Services is offering a transition program for incoming freshmen beginning in July 2023. Please click on Counseling Transition Program for Incoming Freshmen to learn more about this opportunity    

There will be a Parent Orientation Meeting and Student Social on Monday, August 28 at 6:30 PM. More information will be sent out to parents closer to the date.    

Freshman and New Student Orientation is scheduled for Tuesday, August 29, beginning at approximately 8:30 AM and ending at 12:30 PM. Detailed information will be sent out closer to the date.
