Athletics Registration

Spring sports registration opens in Genesis on December 15, 2025. Please complete by the end of February 2026.

Instructions to Complete Sports Registration Forms:

Forms to register your child for a sport will now be completed through Genesis (with the exception of medical clearance forms). If your child would like to participate in a sport, please follow the instructions below:

  1. A parent/guardian must log into parent/guardian account in Genesis to complete forms. (Students cannot register themselves for sports.)
  2. Click on Forms in the left side menu.
  3. Click on Spring Sports Sign-Up & Health History form. Please complete all required fields. This form will be where you select the sport in which your child will participate.
  4. After you submit the Spring Sports Sign-Up form, 24 hours later, you will see additional required athletic forms under the Forms section in Genesis. Please complete the following forms:
    • Emergency Information
    • Concussion Release Form
    • Sudden Cardiac Death Pamphlet and Release
    • Steroid Release
    • Opioid Release and Video
    • Activity Athletic Eligibility
    • Eye Injuries
    • HCRHS Athletic Code of Conduct
    • Impact Concussion - This form has a link for your student to complete Impact baseline concussion testing at home if never done or test has expired. Impact testing is valid for two years. Click here for full Impact testing instructions.
    • Random Drug Testing

Please allow several days for our nurses, athletic trainers, and school doctor to review your forms.  In order to confirm that your child is cleared to participate in the sport, log into your parent Genesis account and click on Athletics on the left side menu. This section will indicate whether your child is cleared for participation or if any requirements are missing.


Medical Clearance and Physicals -

Students wanting to participate in a sport must have a valid Physical Exam on file in the Health Office. If there is a physical on file, the health history update questionnaire and the medical eligibility form are also required for sports participation for each season in which your child participates. Please visit our health services website for the necessary medical forms if you need them. The physical exam and accompanying medical eligibility form is valid for 365 days and MUST be current on the first day of the sports season. For Spring 2026 sports participants, the physical forms must be dated 3/9/25 or later to be valid.  Please email medical forms to the appropriate Health Office Secretary - for 9th & 10th grades, Antoinette Riordan, antoinette.riordan@hcrhs.org, and for 11th & 12th grades, Patricia Mantineo, pmantineo@hcrhs.org, or you may drop them off directly to the appropriate health office. Sports registration forms in Genesis and medical forms need to be submitted well in advance of the sport’s start date, as they must be reviewed/approved before the student is cleared to participate. In addition to the physical exam, if your child has asthma, severe allergies requiring epi-pen, seizures, diabetes, or has had a concussion, additional medical forms are required and are available on our health services website.
