PLEASE NOTE:

New Students: The form titled, "Pre-Participation Physical Evaluation Form" is the correct form to use for 9th grade physicals, new student physicals, and Athletic Pre-participation Physicals.

Athletes: Per State guidelines, all incomplete physical evaluation forms will be returned and must be completed by the physician (medical home) who performed the physical evaluation. Therefore, please review all forms to ensure they are complete prior to submission. Any missing information will result in a return visit to the physician and could result in a delay of student participation in their chosen sport.
